ガイドライン切除例の成績からみた検証

現在の国際診療ガイドラインに基づき，IPMN 149 例の切除成績について検討した．全切除例のうち88％がhigh-risk stigmata，12％がworrisome featuresと術前診断された．切除後にinvasive carcinomaと診断されたのは26％で，全例がhigh-risk stigmataであった．Low-grade dysplasia（42％），high-grade dysplasia（32％）の5年生存率はおのおの91％，98％と良好であったが，invasive carcinomaでは72％と有意に不良であった．分枝型ではworrisome featuresの頻度が27％と高かった．分枝型は主膵管拡張を伴わないため術後膵液瘻の頻度が65％と有意に高く，手術適応の決定に注意が必要と思われた．
